Look yourself up, press the small grey Remove Listing button on your row, and a page reading Removal request has been recorded! replaces the results. Nothing to fill in beyond the search, no email to confirm. The site says removals usually go through automatically within 24 hours. Every row is a listing of its own, and United States Phone Book, run by the same person, needs a separate click.

Three screens
Open americaphonebook.com and fill in First Name, Surname, City and State, then press Search. Only the surname is required, and a city won't run without its two-letter state. To look up a number instead, type all ten digits into Reverse Number.
Each row of results gives a name, an address and a phone number, then a BACKGROUND link and a small grey Remove Listing button at the right. Check the address and number before pressing, since one name can belong to several people, and press the button once on each row that's you.
The Intelius link under the results goes to another company, whose records this removal doesn't touch.
The results give way to Removal request has been recorded! above an empty search form. That's the end of it. No email follows, so screenshot the page if you'd like a dated record.
Afterwards
Give it the day the site quotes, then repeat your search in a private window. If the row is still there, press Remove Listing again. For one that won't budge, lookupuk@gmail.com is the only other channel. The contact page asks people not to email removals, but the privacy policy invites a request that spells out what to remove and on which site.
Both privacy policies file this site under United States Phone Book for emailed requests, yet neither says one button clears both, so look yourself up there too with its removal guide.

The search form, the contact page and the empty results template were rechecked live today, and the template still tells visitors to click REMOVE beside a listing. The recheck swapped a 24 to 48 hour estimate, and a three-business-day figure the site never gave, for the contact page's own 24 hours.
