Fill out the removal search form
Go to truthrecord.org/api/helper/optOutLight/search. The page is titled "Remove My Information" and instructs: "Enter the name and state in the form below to locate the record you would like to remove." Enter the first name, last name, city, and state on the record (zip, phone, and email are optional but tighten the match), then hit SEARCH. Expect CAPTCHAs along the way - this platform serves several back to back in one pass, so keep solving and moving.

Pick your record out of the results
The results page is headed "Select the person you would like to remove" and lists each candidate's name, age, and location history. Check the age and the cities before clicking REMOVE - common names cluster together, and removing the wrong record leaves yours in place.

Fill out the verification form and watch for the email
One last Remove My Information form asks for your email, full name, address, and phone number so they can "verify that you are the matching person" in their system. Fill it in and hit SUBMIT - a masked or secondary email you can open beats your primary here.
The page's own wording: after they receive and verify your information, they send an email within 48 hours, and clicking that email completes the removal. So the request isn't done at SUBMIT - watch the inbox for up to two days, check spam, and click through when it lands.

Give it about 24 hours after you click the verification link, then search your name on the site again - the FAQ suggests clearing your browser history first, since a cached results page can show a record that's already gone. Keep the verification email; it's your proof of submission. The company's own Delete Act filing claims a one-day median for removal requests; that's self-reported, but it matches the stated timeline. If your listing is still up after a few days, email support@truthrecord.org with your submission date and ask for status.

More than one record, or more than one name: the online tool only accepts one record per request - "to prevent fraud," per the FAQ, which gives the example that removing both you and your spouse takes two separate requests. Email support@truthrecord.org for multi-record cleanup.
Record reappears later: the site warns that data partners can re-supply records under differing spellings, and its FAQ names the support email as the channel for records that come back. Refile the opt-out for the new listing, or email support directly.

Refunds only move by mail - print the refund form, complete every field, and send it to the Lauderhill address, because the page warns "Incomplete forms will not be accepted."
